    Depends on your service needs. Mostly, I believe, that any kind of forms is a barrier between your user state "not contacted" to "contacted". In other words less steps better, because it's save ur user time.

    Start with a simple form: "your comment + send button" and add other elements only if it's necessary.

    In this case I believe that the better way to communicate not by forms, but by providing email address. So user can tap and write it's question, where your email address will be automatically provided by the link, as well as users email and his name.
