We're designing a form where users can either enter a phone number or an email address or both. But we have no preference as to which. Has anyone seen any simple examples of this in the real world that don't rely on reams of text to explain?
I know you specifically mentioned that you want to avoid text, but Twitter's solution is perhaps the simplest, most straightforward example of this that I have seen (and I would bet that it's a well-tested solution).

Yes, exactly! But I think the way they present that specific field would work in a non-login form as well.
