Usually walk around the office to see what others are working on and talk to different departments.

This and participating in coffee-related experiments with the engineers. Ya know... for science.
Oh, and we also have a FisherPrice basketball hoop in one of our spare rooms where we "take shots" a couple times a day.
And then there's Friday Game Day where, during lunch, we play a board game. Games played so far:
- Betrayal at the House on the Hill
- Zombie Dice
- Werewolf (aka Mafia, The Village; not a board game)
- And a few others...

What is this "off-time" you speak of? I have heard but whispers of it passing in the night, but I always thought it was a legend or a myth.
I take a walk. I find it incredibly useful to clear my head and come back to work, refocused.
Play Rocket League on PS4. We set one up in the design office.
Work on some side projects, watch some inspirational stuff, try out a new function / tool, chat about something interesting with fellow designers / developers

My offtime is usually reserved for pooping. Or catching up on emails. Or both.
Learn new stuff so you can charge more.
Learning new tech stack and exploring something that is outside of my job scope.
