I wouldn't be surprised if there's a prototype like this but cross my fingers on the fact that they have enough fashion-people in the team. One of them would call this one out "super geeky!" before it went too far.
Still, I'd put my money on "a teenage guy playing with his Cinema 4D" on this.

I want to say that it's a 3D render, but there is clearly a reflection of the person recording it on the device.
I'm sure that could also be faked if he was paying attention to the details.

It's aesthetic language is inline with iPhone 5S and not iPhone 6.

After close inspection of the reflection I'd say, fake!
The camera pans to much with very little shadow changes. Looks the animator added a dummy person to add the shadow but it lacks any movement with the camera.
I hope it's not ;_; it looks ugly.
Since health (and presumably fitness) are supposed to be big parts of the "iWatch" positioning, I can't see this as being it. It looks too uncomfortable to wear for say... a run... or in one of those Apple stock-video-people-in-a-kayak-having-fun moments.
