That's a fake render, and not even a good one.
I think its what authorities use to track people under house arrest.
The music and the "oh s$%" really add to the drama, though.

I want to say that it's a 3D render, but there is clearly a reflection of the person recording it on the device.
I'm sure that could also be faked if he was paying attention to the details.

Joking aside, the reflections look super fake. Pretty certain this isn't real considering fashion bloggers have been invited to this keynote.
Definitely not. Text and icons are way too small to be read at normal watch distance.
Plus, it looks ridiculously uncomfortable.

I'm calling it now: the iWatch won't have a screen. The iPhone already has one.

It's aesthetic language is inline with iPhone 5S and not iPhone 6.

Fake
After close inspection of the reflection I'd say, fake!
The camera pans to much with very little shadow changes. Looks the animator added a dummy person to add the shadow but it lacks any movement with the camera.
I hope it's not ;_; it looks ugly.
Since health (and presumably fitness) are supposed to be big parts of the "iWatch" positioning, I can't see this as being it. It looks too uncomfortable to wear for say... a run... or in one of those Apple stock-video-people-in-a-kayak-having-fun moments.
