Help me out with Common UX Mistakes!

I just started a tumblr where I plan to list common mistakes in UX-design so that we, together, can become better at designing websites and apps.

Tell me what should I cover next, or do you want to help out in another way?

Note that I'm not really a UX-designer, I'm just interested in the subject.

Also, i'm looking for someone to design a tumblr theme so that we don't have to go with the standards.

    Removing or not adding keyboard support (removing outline with css, using other elements then A and BUTTON for interaction, not being able to close modals with ESC and so on).

    Adding maximum-scale=1.0 on viewport .

      I'm with you on everything except maximum-scale=1.0. There are situations where it needs to be set, i.e. if you're trying to get that "appy" feel to the site. Forums and stuff like that should use it, imo.

      It all comes down to user expectation, does it not?

    scroll hijacking, target=_blank on everything, not causing modals/dialogs to close, and speaking of modals– first form input within a modal (or page) needs to be focused upon summon.
